THIS APPLICATION AND OTHER REQUIRED FORMS MUST BE TURNED IN AT TIME OF ENTRY
The Jr. T will follow the T-200 trail to the Oil Well checkpoint, where a mandatory layover will be taken, and return to Mile 112 of the Sterling Highway for the
finish. The entry fee is $50. Each entrant will receive one banquet ticket. PLEASE COMPLETE THE FOLLOWING FORMS AND RETURN with entry fee; Participant's Release (for dog owners other than
musher), Dog Team Application, Health & Medical Treatment Permission, Musher Participant's Release, Musher's Application, Biography, Local Contact, and Sponsor List (if you have
sponsors). Additionally you will need to turn in your birth certificate (or copy) and rookies will need a picture at the same time. THE UNDERSIGNED PARENT OR GAURDIAN HEREBY gives their
permission for first aid treatment to be administered during the Jr. T by designated race officials. I AGREE that I will sign a master copy of the race rules denoting that I have read,
understand and will race under the rules as laid out by the Jr. T. I further agree that I will sign a release (participant's) releasing the Jr. T of all responsibility. These documents must
be signed by the musher and his/her parent(s)/legal guardian(s) in the presence of a notary public or a race official, which ever is required. BY SIGNING THIS APPLICATION the driver agrees
to abide by the rules and policies pertaining to the Jr. T Race as set and determined by Jr. T. The driver and his/her parents/legal guardians also agree to release the Jr. T from all
liabilities which may arise as a result of participation in the Jr. T Race.
